HMRC Starter Checklist

As part of our Employment Checks we require you to download and fill in the following two documents:

• HMRC P46 Starter Checklist – You can complete the HMRC Starter Checklist (HYPERLINK - https://www.gov.uk/government/publications/paye-starter-checklist) on-line or print a copy to fill in. If completing the form on-line, you will still be required to print and sign the document before saving it in an electronic version to upload below.
• Level 1 Disclosure Form - This form can be completed on a PC or Mac. Once completed please save the document and attach it below. If you would like further information in relation to Disclosures, please click on the following link to download a Level 1 Disclosure form

Note: Please complete sections one to five only. Ensure that you complete all fields in each section including your mother’s maiden name and at least one of the documents in section one and in section three. Under sections three and four ensure that you enter the month and year when you commenced your residency at the address/s provided.
